Completing the Florida Health form is an important step in ensuring your child's health needs are met for school entry. The form requires specific information about your child's medical history and evaluation by a health care provider. Follow the steps below to accurately fill out the form.
- Print the form clearly to avoid any confusion.
- Fill in the child’s name (last, first, middle) and birth date.
- Indicate the child’s sex and address, including city and ZIP code.
- Provide a home telephone number for contact purposes.
- Enter the parent/guardian’s name (last, first, middle).
- For Part I, answer questions 1 through 8 by checking “Yes” or “No” in the provided column.
- If you answered “Yes” to any questions, provide explanations in the space provided below.
- Sign and date the form to give permission for the information to be reviewed by school staff.
- In Part II, have a health care provider complete the medical evaluation section, including the date of the exam.
- Ensure the health care provider records screening results, including height, weight, and vision tests.
- Discuss any health conditions that may require emergency action at school with the health care provider.
- Obtain recommendations from the health care provider and ensure they sign and date the form.
Once the form is completed, it should be submitted to the appropriate school personnel. This will help ensure that your child receives the necessary health support during their educational journey.
