Completing the Florida CF 831 form is a crucial step in the certification process for batterer intervention programs. The completed form, along with the required fees and documents, must be submitted to the appropriate department for review.
- Obtain the Florida CF 831 form.
- Check the appropriate box for the type of application: New, Renewal, Change of Ownership, Change of Address, or Change of Director.
- Type or print legibly in the designated areas.
- Fill in the Program Information section, including the Program ID (if applicable), name, street address, judicial circuit, city, county, zip code, number of locations, telephone number, fax number, and email address.
- If applicable, provide the Program Mailing Address.
- List the schedule for group meetings, including locations, days, and times.
- Complete the Onsite Director Information section, including the director's name, professional license number (if applicable), city, county, zip code, telephone number, fax number, and email address. Attach a copy of the director's resume and the CF 1649D form for initial applications.
- Provide information for each facilitator, including their names and professional license numbers (if applicable). Attach necessary documents such as college transcripts and training certificates for new facilitators. Previously approved facilitators do not need to provide attachments for renewal applications.
- Fill out the Applicant Information section, including the applicant's name, position/title, mailing address, and contact details. Attach a copy of the applicant's resume and the CF 1649D form for initial applications.
- Complete the Legal Ownership of BIP section. Choose one category: Individual, Corporation, or Partnership, and provide the required information and attachments for each category.
- Read and acknowledge the declaration statement regarding the accuracy of the information provided, understanding of the rules, and consequences of misrepresentation.
- Sign and date the application.
- Mail the completed application, along with the application fee (check payable to the Department of Children & Families) and any required documents, to the specified address.
